        <![CDATA[<div>Sweden is in the midst of a healthcare reform. The aim is to achieve a shift from a traditional healthcare system focused on emergency hospitals and specialist care to healthcare provision closer to the patients with increased patient involvement and more preventative measures. By reorganizing and using the opportunities that new technology and digitalization provide, advanced care can be provided outside hospitals. Local clinics and your own home will replace hospital wards. Some of the Swedish regions have come far in this work, the vaste and sparsely populated Jämtland-Härjedalen is one of them. <br>
<br>
In this episode we will look into how remote patient monitoring can transform healthcare systems and handle more patients with fewer resources but equally important, how it empowers patients and improves their quality of life.<br>
